*Passports issued to minors up to age 16 cannot be renewed. Minors must use the first time application to apply for a new passport.

Adults

  • Completed form DS-11 passport application
  • Proof of U.S. Citizenship (one of the following)
    • A certified copy of U.S. birth certificate (must include parents' names; birth cards not accepted)
    • Certificate of naturalization
    • Expired passport (not eligible for renewal)
  • Proof of identity (one of the following)
    • Valid driver’s license
    • Valid passport
    • Valid military ID
    • Certificate of naturalization
  • One recent color photograph
    • This service is not offered at Clerk's Office
    • Get photo made at any business that offers this service
    • Must be 2x2 inches in size

Fees

  • $35 to the Clerk of Court (book and/or card). Acceptable forms of payment include cash, check and money order.  Visa and MasterCard, debit and credit cards are subject to a 3% convenience fee.
  • $110 (check or money order) to the U.S. Department of State (book only)
  • $30 (check or money order) to the U.S. Department of State (card only)
  • $140 (check or money order) to the U.S. Department of State (book and card)
  • Regular service four to six weeks, expedited service two to three weeks

 

    Minors

    • Completed Form DS-11 passport application
    • Child must be present at the time of application
    • Both parents must be present at the time of application or
      • One parent may apply and submit a consent form signed and notarized by the other parent.
    • Proof of identity for both parents or applying parent (one of the following)
      • Valid driver’s license 
      • Valid passport 
      • Valid military ID
      • Certificate of naturalization
    • Proof of U.S. citizenship for the minor (one of the following)
      • A certified copy of U.S. birth certificate
      • Certificate of naturalization
    • One recent color photograph

    Fees

    • $15 (check or money order) to the U.S. Department of State (card only)
    • $95 (check or money order) to the U.S. Department of State (book and card)
    • Regular service four to six weeks, expedited service two to three weeks.  $60 expedited fee + $14.85 return shipping fee per expedited passport (check or money order).

     

      The busy season starts in spring for passport requests. If traveling in less than four weeks, expedite services to avoid any delays in application processing.

      Renewals

      • Only adult passports can be renewed
      • Passports can be renewed up to five years after expiration.
      • The Clerk’s office does not process this application but will be happy to assist with information. This application must be mailed directly by the applicant to the passport office.
      • Completed form DS-82 passport application
      • One recent color photograph
        • This service is not offered at Clerk's Office
        • Get photo made at any business that offers this service
        • Must be 2x2 inches in size
      • Current passport
      • If your name has changed since the issuance of your current passport, submit proof of name change (i.e. certified copy of marriage license, certified copy of court order, etc.)
      Fees
      • $110 to the U.S. Department of State (check or money order only)
      • Regular service four to six weeks, expedited service 2 to 3 weeks
        • Expedited service (16 and older). In addition to the above requirements, include:
          • All fees to the U.S. Department of State can be on the same check or money order.
          • $60 (expedite fee) plus $110 to the U.S. Department of State
          • Overnight postage, $15.89 to the U.S. Department of State. Your passport must be mailed overnight at the time of application.
          • If the passport is needed in less than two to three weeks, applicants are strongly encouraged to apply in person at the U.S. Department of State.
