Family Assessment and Planning Team
The Family Assessment and Planning Team (FAPT) is comprised of the supervisory staff from the local Department of Social Services, school system, Community Services Board (mental health) and the Court Services Unit (juvenile justice), as well as a parent
and representatives from private providers in the community. The FAPT works with the families to develop an individualized family service plan or plan of care. This team makes recommendations for strategies and services based on the strengths and
needs of the family.

CSA Parent Representative Application
Montgomery County actively recruits for parent representatives for the Family Assessment & Planning Team (FAPT) and the Community Policy & Management Team (CPMT). A parent representative cannot be an employee of any public or private program
which serves children/families, and must have a child who has received services that are within the purview of CSA. Please be advised that a parent representative must complete a Statement of Economic Interest/Conflict of Interest Form for appointment.
Parent representatives are appointed by Board of Supervisors to the CPMT. If interested, complete the application and send it to Montgomery County CSA, 210 S. Pepper St., Ste. D, Christiansburg, VA 24073.
